Описание
Таблетки круглой формы с плоской поверхностью светло-желтого (дневные) и светло-зеленого (ночные) цвета, с риской на одной стороне и гравировкой «Medley» на другой стороне.

Фармакологические свойства
Фармакокинетика
После приема внутрь парацетамол быстро абсорбируется из желудочно-кишечного тракта (ЖКТ), преимущественно в тонком кишечнике. После однократного приема в дозе 500 мг максимальная концентрация в плазме крови (Сmax) достигается через 10-60 мин и составляет около 6 мкг/мл, затем постепенно снижается и через 6 ч составляет 11-12 мкг/мл. Связывание с белками крови составляет менее 10%. Выводится с мочой главным образом в виде глюкуронидных и сульфатных конъюгатов.
Кофеин метаболизируется в печени (97%) в 1- 7-метилксантин, 1,7-диметилметилксантин и 1,3-диметилмочевой кислоты, которые выводятся с мочой.
После приема внутрь фенилэфрин плохо всасывается из ЖКТ. Метаболизируется при участии МАО в стенке кишечника и при «первом прохождении» через печень. Биодоступность фенилэфрина низкая.
Хлорфенирамин хорошо проникает в разные ткани и через гематоэнцефалический барьер. Метаболизируется в печени путем метилирования. Выводится в виде метаболитов и в неизмененном виде почками и желудочно-кишечным трактом (ЖКТ).
Фармакодинамика
Колдфри® комбинированный препарат обладает анальгезирующим, жаропонижающим и противоаллергическим действиями, обусловленными активными веществами, входящими в состав препарата.
Парацетамол – нестероидное противовоспалительное средство, оказывает жаропонижающее, анальгетическое и противовоспалительное действие.
Хлорфенирамина малеат – антигистаминное средство, антагонист Н1-гистаминовых рецепторов, оказывает противоотечное действие.
Фенилэфрина гидрохлорид – селективный агонист ά1-адренорецепторов, оказывает сосудосуживающее действие, способствуя тем самым устранению заложенности носа и уменьшению выделений из носа.
Кофеина ангидрид – психостимулирующее средство, производное пурина, повышает умственную и физическую работоспособность, снижая усталость и сонливость. Уменьшает сонливость, вызванную хлорфенирамином.
Показания к применению
Для устранения заложенности носа, выделений из носа, чихания, головной боли, болей в теле и конечностях, высокой температуры при:

Способ применения и дозы
Взрослым: 1 светло-желтая таблетка (Колдфри® дневные таблетки) – утром и днем, 1 светло-зеленая таблетка (Колдфри® ночные таблетки) – на ночь.
Таблетки принимают в течении 3-х дней, запивая водой, фруктовым соком или молоком. Не рекомендуется превышать указанные дозы и принимать Колдфри® ночные таблетки в течение дня.
Побочные действия
— кожная сыпь, зуд, крапивница, ангионевротический отек
— головокружение, нарушение засыпания, повышенная возбудимость
— повышение артериального давления, тахикардия
— тошнота, рвота, эпигастральная боль, сухость во рту
— задержка мочи
— парез аккомодации, повышение внутриглазного давления, мидриаз
— анемия, тромбоцитопения, агранулоцитоз, гемолитическая анемия, апластическая анемия, метгемоглобинемия, панцитопения
— гепатотоксическое действие
— нефротоксичность (папиллярный некроз)
— бронхообструкция.
— при длительном применении, особенно в больших дозах, возможно развитие метгемоглобинемии, анемии, нефротоксического и гепатотоксического действия.
С осторожностью – выраженный атеросклероз коронарных артерий, артериальная гипертензия, тиреотоксикоз, феохромоцитома, сахарный диабет, бронхиальная астма, хроническая обструктивная болезнь легких, дефицит глюкозо-6-фосфатдегидрогеназы, заболевания крови, врожденные гипербилирубинемии (синдромы Жильбера, Дубина-Джонсона и Ротора), печеночная и/или почечная недостаточность, закрытоугольная глаукома, гиперплазия предстательной железы.

— одновременный прием трициклических антидепрессантов, ингибиторов МАО, бета-адреноблокаторов
— одновременный прием препаратов, содержащих парацетамол
— артериальная гипертензия
— гипертиреоз

— детский возраст до 18 лет
Лекарственные взаимодействия
Препарат может усиливать эффекты ингибиторов МАО (моноаминооксидазы), трициклических антидепрессантов, седативных препаратов, этанола и этанолсодержащих препаратов.
Этанол стимулирует метаболизм парацетамола и повышает его гепатоксичность
При одновременном применении парацетамола с азидотимидином возможно развитие нейтропении.
При одновременном применении с хлорамфениколом парацетамол повышает его токсичность.
Препараты, стимулирующие активность микросомальных изоферментов, усиливают гепатотоксическое действие парацетамола.
Особые указания
В период лечения следует воздержаться от приема алкоголя, снотворных и анксиолитических (транквилизаторы) лекарственных средств. Не принимать вместе с другими лекарственными средствами, содержащими парацетамол. Если симптомы заболевания не проходят в течение 3 – 5 дней, обратиться к врачу.

Следует соблюдать осторожность при выполнении работы, требующей концентрации внимания, повышенной скорости двигательных и психических реакций.
Передозировка
Симптомы: сухость во рту, учащение пульса, аритмии, потливость, тошнота, рвота, боли в животе, повышенная активность (перевозбуждение), галлюцинации, острая печеночная и/или почечная недостаточность.
Лечение: промывание желудка, внутривенное введение ацетилцистеина или метионина, симптоматическое лечение.
Форма выпуска и упаковка
По 6 таблеток (дневные) и 3 таблетки (ночные) в контурную ячейковую упаковку из пленки поливинилхлоридной и фольги алюминиевой печатной лакированной.
По 1 контурной ячейковой упаковке вместе с инструкцией по медицинскому применению на государственном и русском языках помещают в пачку из картона.

Kufril syrup is an active mucolytic agent that treats coughs with mucus. Its ingredients are Ambroxol (15 mg), Levosalbutamol (0.5 mg) and Guaifenesin (50 mg). Ambroxol is a mucokinetic agent that makes the sputum thinner and easily removable by coughing.
Levosalbutamol is an adrenergic receptor antagonist that smoothes the respiratory muscles and eases air in and out of the lungs. Guaifenesin is an expectorant; it clears chest congestion. It thins the mucus in the air passageway to make it easier to cough up the mucus and decongest the chest.
It is used in the treatment of cough, acute and chronic bronchopulmonary problems, abnormal mucus secretion, chest congestion, thick mucus, constricted airways, inflammation of lungs and sore throat.
It is an anti-inflammatory and anti-oxidant anaesthetic which improves the patient’s conditions by relaxing the muscles in the airways, thinning mucus and reducing phlegm in the respiratory passage.
For adults, the usual dosage is 30 to 120 mg per day, taken twice or thrice a day, according to the doctor’s recommendation. It is usually recommended to take the syrup after food.

It is an agent that causes diffusion of thick mucus. Uncontrolled quantity of nitric oxide (NO) is related to inflammation and other kinds of disruptions of the functioning of the airways. It inhibits the activation of guanylate cyclase which is nitric oxide dependent. Simultaneously, it works as a β2 adrenergic receptor that results in increase of cyclic AMP. Increased AMP leads to activation of protein kinase A which again prevents phosphorylation of myosin. All this mechanism leads to relaxation of smooth muscles. It also reduces adhesiveness and surface tension of congealed mucus in upper respiratory tract to increase flow of mucus, stimulating and increasing efficiency of the cilia to remove the accumulated mucus.

Kufril Ls Drops
Ambroxol is a metabolite of Bromhexine. It possesses mucokinetic (improvement in mucus transport) and secretolytic (liquefies secretions) properties. Ambroxol stimulates the serous cells of the glands of the mucous membrane of bronchi, increasing the content of mucus secretion. The mucolytic effect is associated with depolymerization and splitting of mucoproteins and mucopolysaccharide fibres, which leads to reduction in the viscosity of mucus. Expectoration of mucus is facilitated and breathing is eased considerably. Ambroxol stimulates production of phospholipids of surfactant by alveolar cells. Ambroxol has anti-inflammatory properties. In patients with COPD, it improves airway patency. Beside these, Ambroxol also exhibits anti-oxidant activity. Long-term use is possible because of the good tolerability of the preparation.

How Kufril Ls works
Ambroxol is a mucolytic agent. Excessive Nitric oxide (NO) is associated with inflammatory and some other disturbances of airways function. NO enhances the activation of soluble guanylate cyclase and cGMP accumulation. Ambroxol has been shown to inhibit the NO-dependent activation of soluble guanylate cyclase. It is also possible that the inhibition of NO-dependent activation of soluble guanylate cyclase can suppress the excessive mucus secretion, therefore it lowers the phlegm viscosity and improves the mucociliary transport of bronchial secretions.

Average daily dose (preferably after meal):Pediatric Drops:
- 0-6 months: 0.5 ml 2 times a day
- 6-12 months: 1 ml 2 times a day
- 1-2 years: 1.25 ml 2 times a day
Syrup:

Side Effects
Gastrointestinal side-effects like epigastric pain, gastric fullness may occur occasionally. Rarely allergic responses such as eruption, urticaria or angioneurotic edema may occur.

Precaution
Ambroxol should be given cautiously to patients with gastric and duodenal ulceration or convulsive disorders. Patients with hepatic and renal insufficiency should take it with caution.
Interaction
Ambroxol has no interaction with cardioactive glycosides, corticosteroids, bronchodilators, diuretics and antibiotics (normally used in the treatment of bronchopulmonary affections). But Ambroxol should not be taken simultaneously with antitussives (e.g. Codeine) because mucus, which has been liquefied by Ambroxol, might not be expectorated.

Kufril-LS Expectorant is a combination of Levosalbutamol, Ambroxol and Guaifenesin.This medicine is used in relieving cough with mucous. This medicine is also used in the treatment of bronchitis, asthma, and other breathing disorders by relieving symptoms such as wheezing, shortness of breath, chest tightness, breathing difficulties, coughing, etc. Kufril-LS Expectorant dissolves thick mucous formed in the air passages and makes it easier to cough out the mucous formed in your chest. It also clears and widens your airway tubes which makes it easy for you to breathe.
Kufril-LS Expectorant shows some side effects like headache, diarrhoea, stomach upset, dizziness, nausea, vomiting, etc. Most of these are short-lasting and usually get better after some time. Talk to your doctor if these side effects bother you. This medicine may cause sleepiness, so you should avoid activities requiring mental focus like driving and operating machines.
Kufril-LS Expectorant can be taken with or without food as prescribed by your doctor. Take this medicine preferably with food to avoid any stomach problems.
