В/м, глубоко. При остеопорозе — 50 мг 1 раз в 3 нед, при диссеминированной карциноме молочных желез у женщин — 50 мг 1 раз в 2–3 нед, при состояниях, связанных с потерей белка, — по 25–50 мг 1 раз в 3 нед.
В/м, глубоко. Взрослым средняя разовая доза — 25–50 мг каждые 3–4 нед. При миопатиях могут быть установлены индивидуальные дозы. При тяжелой почечной недостаточности — 50 мг еженедельно. Онкологическим больным — 50 мг каждые 5 дней.
Детям — 400 мкг/кг каждые 3–4 нед.

Medical examination:
Physicians should consider monitoring patients receiving Zebol-50 before the start of treatment, at quarterly intervals for the first 12 months and yearly thereafter for the following parameters:
— Hematocrit and hemoglobin to exclude polycythemia.
Conditions that need supervision:
Patients, especially the elderly, with the following conditions should be monitored for:
— Tumours — Mammary carcinoma, hypernephroma, bronchial carcinoma and skeletal metastases. In these patients hypercalcaemia or hypercalciuria may develop spontaneously, and also during androgen therapy. Nevertheless, the hypercalcaemia or hypercalciuria should first be treated appropriately and after restoration of normal calcium levels, if judged necessary and taking into account the risks and benefits on a case by case basis, hormone therapy can be resumed, with caution.
— Pre-existing conditions-In patients with pre-existing cardiac, renal or hepatic insufficiency/disease or epilepsy or migraine anabolic steroid treatment may cause complications characterized by oedema with or without congestive heart failure. In such cases treatment must be stopped immediately. Patients who experienced myocardial infarction, cardiac-, hepatic- or renal insufficiency, hypertension, epilepsy, or migraine should be monitored due to the risk of deterioration of or reoccurrence of disease. In such cases treatment must be stopped immediately.
— Diabetes mellitus — Zebol-50 can improve glucose tolerance in diabetic patients.
— Anti-coagulant therapy — Zebol-50 can enhance the anti-coagulant action of coumarin-type agents (see also section 4.5).
— Liver dysfunction — caution should be used in patients with severe hepatic impairment and Zebol-50 50mg/ml should only be used if the benefits outweigh the risks.
Adverse events:
If anabolic steroid-associated adverse reactions occur , treatment with Zebol-50 should be discontinued and, upon resolution of complaints, treatment can be resumed.
Virilisation:
Patients should be informed about the potential occurrence of signs of virilisation. In particular, singers and women with speech professions should be informed about the risk of deepening of the voice.
If signs of virilisation develop, the risk/benefit ratio has to be newly assessed with the individual patient.
(Mis) use in sports:
Nandrolone is classified as a prohibited substance under the Olympic Movement Anti- doping Code (OMAC 1999). The misuse of Nandrolone and other anabolic steroids to enhance ability in sports carries serious health risks and is to be discouraged.
Excipients:
Zebol-50 contains arachis oil (peanut oil) and should not be taken/applied by patients known to be allergic to peanut. As there is a possible relationship between allergy to peanut and allergy to soya, patients with soya allergy should also avoid Zebol-50.
Zebol-50 50mg/ml contains 100 mg benzyl alcohol per ml solution and must not be given to premature babies or neonates. Benzyl alcohol may cause anaphylactoid reactions in infants and children up to 3 years old.
Paediatric Population:
Safety and efficacy have not been adequately determined in children and adolescents. In pre-pubertal children statural growth and sexual development should be monitored since anabolic steroids in general and Zebol-50 in high dosages may accelerate epiphyseal closure and sexual maturation.

Due to the nature of Zebol-50, side effects cannot be quickly reversed by discontinuing medication. Injectables in general, may cause local reaction at the injection site.

Nandrolone is chemically related to testosterone and shows enhanced anabolic and a reduced androgenic activity.
In humans Zebol-50 has been shown to positively influence calcium metabolism and to increase bone mass in osteoporosis.
Androgenic effects (e.g. virilisation) are relatively uncommon at the recommended dosages. Nandrolone lacks the C17 alpha-alkyl group which is associated with the occurrence of liver dysfunction and cholestasis.

Absorption
Nandrolone decanoate is slowly released from the injection site into the blood with a half-life of 6 days.
Distribution
The ester is rapidly hydrolysed to nandrolone in the blood with a half-life of one hour or less. The half-life for the combined process of hydrolysis of nandrolone decanoate and of distribution and elimination of nandrolone is 4.3 hours.
Biotransformation and excretion
Nandrolone is metabolised by the liver. 19-norandrosterone, 19-noretiocholanolone and 19-norepiandrosterone have been identified as metabolites in the urine. It is not known whether these metabolites display a pharmacological action.

Toxicity studies in animals after repeated dosing did not indicate a safety risk for humans. No formal studies to assess reproduction toxicity, genotoxicity and carcinogenicity have been conducted by the company. As a class, anabolic steroids are considered to be probably carcinogenic to humans (IARC Group 2a).
The use of androgens in different species has resulted in virilisation of the external genitals of female foetuses. Investigations into the genotoxic potential of nandrolone showed it to be positive in an in vitro micronucleus assay and an in vivo micronucleus assay in mouse but not rat, and in the comet assay of mouse and rat. The clinical relevance of these findings is unknown, therefore the risk to patients cannot be ruled out.

Анемия
Взрослые:
Мужчины: 200 мг в неделю, внутримышечно
Женщины. 100 мг в неделю, внутримышечно.
В начале лечения могут наблюдаться значимые индивидуальные вариации. Если результат лечения не удовлетворителен через 3-6 месяцев от начала лечения, следует приостановить введение Ретаболила.
После нормализации или значительного повышения количества эритроцитов, следует постепенно снижать дозу препарата, параллельно с регулярным мониторированием гематологических показателей. В случае рецидива на фоне снижения дозы препарата или после отмены препарата Ретаболил, следует рассмотреть повторный курс лечения.
Период реконвалесценции (выздоровления).
Взрослые:
50 мг внутримышечно с 3- 4-недельными интервалами.
Способ введения: глубоко внутримышечно.
Инъекции препарата Ретаболил не рекомендуется применять у детей и подростков младше 18 лет, вследствие недостаточного количества данных по эффективности и безопасности.
Гиперчувствительность к действующему веществу или любому из компонентов препарата. Беременность, лактация.
Карцинома предстательной железы или молочной железы у мужчин.
Особые предупреждения и меры предосторожности при применении
Не превышать рекомендованные дозы.
С учетом андрогенного нежелательного воздействия, следует назначать Ретаболил женщинам только после оценки ожидаемой пользы и потенциального риска. Следует соблюдать осторожность при лечении пациентов с дисфункцией миокарда, артериальной гипертензией, печеночной или почечной недостаточностью, а также эпилепсией, мигренью или глаукомой в настоящий момент или в анамнезе.
Во время лечения препаратом Ретаболил рекомендуется регулярно контролировать внутриглазное давление, так как имеется риск задержки натрия и воды.
Могут иметь место нарушения функции печени (например, задержка бромсульфталеина — BSP), поэтому необходимо провести исследование функции печени через 4 недели от начала лечения.
Мужчинам необходимо пройти пальцевое исследование прямой кишки перед началом лечения препаратом Ретаболил и проводить его периодически во время лечения.
Необходимо скорректировать дозу гипогликемических препаратов.
У пациентов со злокачественными заболеваниями продолжительность лечения определяется индивидуально, также как и в случае изменения результатов функциональных печеночных тестов.
Ретаболил масляный раствор для инъекций, содержит 0.05 мл (52.25 мг) бензилового спирта и 0.2 мл изопропилового спирта в 1 мл раствора, поэтому Ретаболил нельзя назначать пациентам с аллергией на бензиловый спирт. Количество изопропилового спирта клинически не значимо.
Взаимодействие с другими лекарственными препаратами и другие формы взаимодействия
Одновременное применение препарата Ретаболила и перечисленных далее препаратов требует осторожности:
Гипогликемические препараты
Производные кумарина, так как нандролон может усиливать эффекты этих препаратов.
Беременность и лактация
Препарат Ретаболил противопоказан во время беременности и лактации.
Исследований влияния препарата Ретаболил на способность управлять транспортными средствами и работать с механизмами не проводилось.
Побочные реакции
Ретаболил масляный раствор для инъекций при использовании в терапевтических дозах обычно не вызывает побочных реакций.
При продолжительном лечении могут наблюдаться следующие побочные реакции, классифицированные в соответствии с системно-органной классификацией. Данные по частоте отсутствуют.
Доброкачественные, злокачественные и неуточнённые новообразования, включая кисты и полипы
Опухоли печени*
Нарушения со стороны иммунной системы.
Реакции гиперчувствительности
Нарушения со стороны эндокринной системы
Ингибирование секреции гонадотропина.
У женщин может иметь место вирилизация (например, акне, гирсутизм, облысение по мужскому типу, необратимое огрубение голоса, нарушения менструального цикла, гипертрофия клитора).
Охриплость голоса может являться первым признаком изменения тональности, которое может быть длительным, а иногда необратимым.
Аменорея и ингибирование сперматогенеза.
Гинекомастия у мужчин.
Изменение либидо.
У мальчиков препубертатного возраста может наблюдаться учащение эрекций и увеличение пениса, у девочек — усиление роста лобковых волос и гипертрофия клитора.
Нарушения со стороны желудочно-кишечного тракта
Тошнота, рвота, снижение аппетита.
Нарушения со стороны печени и желчевыводящих путей
Холестаз, желтуха.
Нарушения со стороны кожи и подкожно-жировой клетчатки
Акне, слабовыраженный гирсутизм.
Нарушения со стороны почек и мочевыводящих путей
Задержка натрия и воды.
Общие расстройства и нарушения в месте введения
Отек.
Данные лабораторных и инструментальных исследований
Повышение концентрации мочевины в сыворотке крови, положительный азотистый баланс, гиперкальциемия (в особенности у пациентов, соблюдающих строгий постельный режим, и у женщин с метастазирующим раком молочной железы). Отклонение (повышение) показателей функциональных печеночных тестов.
* При длительном применении внутрь активных С17-альфа-алкилированных анаболических стероидов наблюдались в редких случаях опухоли печени. Связь между появлением опухолей печени и инъекционным введением не-С 17-алкилированных стероидов, таких, как эфиры нандролона, крайне маловероятна, но, тем не менее, ее нельзя полностью исключить.
Данных по передозировке препарата Ретаболил не сообщалось.
Злоупотребление препаратами, содержащими нандролон (используются в высоких дозах в качестве анаболиков) связано с тяжелыми гормональными (эндокринными), метаболическими и психическими побочными реакциями.

Фармакологические свойства
Фармакодинамические свойства
Действующее вещество препарата Ретаболил масляный раствор для инъекций, нандролон, представляет собой производное тестостерона с усиленным анаболическим действием и сниженным андрогенным эффектом. Эти эффекты были продемонстрированы в исследованиях на животных и на рецепторах, а также в процессе клинического применения препарата.
В исследованиях на животных было показано, что нандролона деканоат стимулирует эритропоэз, благодаря, с одной стороны, прямой стимуляции эритропоэтических стволовых клеток костного мозга, и с другой стороны — усиленному высвобождению эритропоэтина. Нандролон защищает костный мозг от супрессивного действия цитотоксических агентов.
У человека нандролона деканоат стимулирует эритропоэз, о чем свидетельствует увеличение числа эритроцитов, гематокрита и содержания гемоглобина. Этот эффект препарата важен при лечении анемий, вызванных дефицитом синтеза эритропоэтина, а также анемий, вызванных супрессией костного мозга цитотоксическими препаратами для химиотерапии, или недостаточностью по причине гипоплазии. При апластической анемии эритропоэтический ответ обычно сопровождается увеличением лейкопоэза и тромбопоэза. Андрогенные нежелательные реакции (например, вирилизация) при использовании препарата в рекомендованных дозах наблюдаются нечасто. Нандролон не имеет в своем составе С17 α-алкильную группу, с которой могут быть связаны нарушения функции печени и холестаз.
Фармакокинетические свойства
Нандролона деканоат медленно высвобождается из места инъекции в системный кровоток, период полувыведения составляет 6 дней. Эфир быстро гидролизируется до нандролона в кровотоке, период полувыведения которого составляет 1 час или менее. Суммарный период полувыведения с учетом процесса гидролиза нандролона деканоата и распределения и выведения нандролона в общей сложности составляет 4.3 часа.
Нандролон метаболизируется в печени до метаболитов 19-норандростерона, 19-норэтиохоланолона и 19-норэпиандростерона, которые определяются в моче. Неизвестно, обладают ли эти метаболиты каким-либо фармакологическим действием.

Фармакология:
Zebol-50 — инъекционный анаболический стероид пролонгированного действия. Является производным тестостерона. Стимулирует синтез белка в организме, способствует задержке в организме азота, фосфора, серы, калия. Способствует фиксации кальция в костях, увеличению костной массы при остеопорозе. Вызывает повышение аппетита, увеличение массы мышц с соответствующим увеличением массы тела. У женщин с диссеминированной карциномой молочных желез способствует улучшению состояния в течение многих месяцев. Характеризуется длительным действием. Обладает низкой андрогенной активностью. После инъекции эффект наступает в первые 3 дня, достигает максимума к 7-му дню и продолжается не менее 3 недель.
Показания:
• Кахексия различной этиологии;
• Астения;
• Остеопороз;
• Необходимость ускорения регенерации (в т.ч. при переломах, травмах);
• Нарушения белкового обмена после тяжелых травм, операций, ожогов, лучевой терапии,
• При инфекционных заболеваниях, сопровождающихся потерей белка (туберкулез и т. д.);
• При диабетической ретинопатии, нефропатии;
• Стимуляция роста у детей с задержкой роста;
• Необходимость длительной анаболической терапии при прогрессирующей мышечной дистрофии, остеопорозе и раке;
• Диссеминированный рак молочных желез у женщин.

Режим дозирования:
Zebol-50 — вводят глубоко в/м.
Взрослым по 25-100 мг I раз в 1-3 недели. При тяжелой почечной недостаточности по 50 мг 1 раз в неделю. При
нарушении белкового обмена- 25 мг в/м 1 раз/неделю
Детям назначают в дозе 400 мкг/кг массы тела 1 раз в 3-4 недели
Противопоказания:
• Рак предстательной железы
• рак грудной железы у мужчин
• гепатит и острая дистрофия печени
• беременность и период лактации.
Побочные действия:
Побочное действие вероятно при применении нандролона в высоких дозах.
Со стороны эндокринной системы вирилизация у девочек, женщин и мальчиков в препубертатном периоде, аменорея, нарушение сперматогенеза
Со стораны Обмена веществ возможно преждевременное окостенение эпифизов и задержка роста.

Zebol-50 Phenylpropionate is an injectable anabolic preparation. The pharmacologically active substance is nandrolone. The phenylpropionate ester gives the preparation a duration of action of about one week after injection. Zebol-50 is chemically related to the male hormone. Compared to testosterone, it has an enhanced anabolic and a reduced androgenic activity. This has been demonstrated in animal bioassays and explained by receptor binding studies. The low androgenicity of nandrolone is confirmed in clinical use.In the human, Zebol-50 Phenylpropionate has a nitrogen saving action. This effect on protein metabolism has been established by metabolic studies and is utilized therapeutically in conditions where a protein deficiency exists such as during chronic debilitating diseases and after major surgery and severe trauma. In these conditions, Zebol-50 Phenylpropionate serves as a supportive adjunct to specific therapies and dietary measures. In women with disseminated mammary carcinoma, in selected cases Zebol-50 Phenylpropionate has been reported to produce objective regressions for many months. Furthermore, Zebol-50 Phenylpropionate has been shown to produce relief of bone pain in patients with osteoporosis. The similarly acting nandrolone ester, nandrolone decanoate, has been shown to increase bone mineral content in these patients. Androgenic effects (e.g. virilisation) are relatively uncommon at the recommended dosages. Zebol-50 lacks the C17alpha-alkyl group which is associated with the occurrence of liver dysfunction and cholestasis.

Dosage
Zebol-50 dosage
As an adjunct to specific therapies and dietary measures in pathologic conditions characterized by a negative nitrogen balance: Recommended dose is 25-50 mg every week.
For the palliative treatment of selected cases of disseminated mammary carcinoma in women: Recommended dose is 50 mg every week.
Osteoporosis: Recommended dose is 50 mg every week.
For an optimal therapeutic effect it is necessary to administer adequate amounts of vitamins, minerals and protein in a calorie-rich diet. Zebol-50 Phenylpropionate should be administered by deep intramuscular injection.

Side Effects
High dosages, prolonged treatment and/or too frequent administration may cause:
- Virilisation which appears in sensitive women as hoarseness, acne hirsutism and increase of libido; in prepubertal boys as an increased frequency of erections and phallic enlargement, and in girls as an increase of pubic hair and clitoral hypertrophy. Hoarseness may be the first symptom of vocal change which may end in a long-lasting, sometimes irreversible deepening of the voice.
- Amenorrhoea.
- Inhibition of spermatogenesis.
- Premature epiphyseal closure.
- Fluid retention.

Precaution
If signs of virilisation develop, discontinuation of the treatment should be considered, Preferably in consultation with the patient.
It is recommended to monitor patients with any of the following conditions:
- latent or overt cardiac failure, renal dysfunction, hypertension or migraine (or a history of these conditions), since anabolic steroids may occasioally induce fluid retention:
- incomplete statural growth, since anabolic steroids in high dosages may accelerate epiphyseal closure;
- skeletal metastases of breast carcinoma. In these patients
- hypercalcaemia may develop both spontaneously and as a result of anabolic steroid therapy. The later can be indicative of a positive tumour response to the hormonal treatment. Nevertheless, the hypercalcaemia should first be treated appropriately and after restoration of normal calcium levels hormone therapy can be resumed;
- liver dysfunction.
The use of anabolic steroids to enhance athletic ability may carry severe risks to the user’s health and should be discouraged.
Interaction
Anabolic steroids may improve glucose tolerance and decrease the need for insulin or other antidiabetic medicines in diabetics.
Zebol-50 Cholesterol interaction
[Major] Androgenic anabolic steroids may adversely affect serum lipids, including lowering HDL and elevating LDL levels.
These changes can be marked, particularly with the 17-alpha-alkyl derivatives (i.e., fluoxymesterone, methyltestosterone, oxandrolone, oxymetholone, and stanozolol), and may significantly impact the risk of atherosclerosis and coronary artery disease.
Patients with preexisting hyperlipoproteinemia may require closer monitoring during therapy with androgenic agents, and adjustments made accordingly in their lipid-lowering regimen.
Androgen therapy should be administered cautiously in patients with coronary artery disease or a history of ischemic heart disease.
